Optometry Unit

The Sangre Grande Optometry Unit function is to manage ophthalmic referrals from the community health system. It is NOT a centre for refraction and dispensing of spectacles, which is well served by community optometrists.

Eye Clinic appointments are arranged by way of faxed/posted referral letters from the following authorised centres:

ERHA Heath Centre Doctor
Sangre Grande A & E Department
Community Optometrists
Ophthalmologists
Medical Practitioners
Other Public Health Institutions

Patients: Please do not try and jump the system by self referral. If you simpy turn up to a clinic,  you will not be seen. This clinic runs ONLY on referrals from professionals.

Referral letters are assessed in the Unit as to type of referral and level of urgency and the patient is then allocated to the relevant clinic. Patients are then directly contacted by the Unit to arrange appointments. This process takes approximately 2 weeks and routine appointments are usually arranged within 3 to 4 months.

Draft Referral Forms
Standard forms are being developed to improve speed and efficiency of referrals. These are available below in DRAFT form only for evaluation and feedback and should not be used in for any actual referrals.
